Percentage of untranfused patients increased from 9% in group 1 to 31% and 22% in groups 2 and 3, respectively. Secondly, increasing numbers of transfused patients received blood products from which antigen-presenting leukocytes have been removed leukopoor ; and which have been irradiated in vitro before administration. Both manipulations reduced the risk of graft rejection in animal studies.36-38 Thirdly, most transfused patients in group 2 and many of those in group 3 through June 1988 ; received donor buffy coat cell infusions in addition to the marrow graft, which were administered in an attempt at increasing the number of transplanted stem cells.39 Buffy coat cell infusions were discontinued in mid-1988 because of the unusually high incidence of chronic GVHD seen with this treatment modality.40 Fourthly, there have been improvements in the immunosuppressive quality of conditioning programs. For example, beginning in July 1988, all patients transplanted at FHCRC have been conditioned for their first graft with a combination of CY ATG, 19 which was known to be an effective conditioning program for second transplants.27 Other centers introduced radiation in the conditioning regimens, either in the form of TBI or partial body irradiation such as total lymphoid or thoraco-abdominal41 irradiation. Although effective in reducing rejection, 17, 42-44 the inclusion of irradiation may have increased the risks of GVHD, 18, 42 interstitial pneumonia, 42, 45 and secondary cancers, compared with CY-based regimens.46 Also, growth, development, and fertility may be impaired in irradiated patients.47-49 By comparison, no unusual short- or long-term side effects have been observed as yet with the CY ATG regimen.32 Almost concurrent with the progressive decrease in rejection rates, survivals after second transplants increased. One important factor contributing to this success has been a significant delay in the time to rejection of the first graft from a median of 28 days among group 1 patients to 180 days in group 3 patients. In direct relation to the times to rejection, the intertransplant intervals increased. Patients undergoing second transplants after a longer intertransplant interval had recovered from toxicities associated with the first graft, and they were in much better clinical condition than those transplanted sooner after the first graft. Consequently, the patients were less likely to die from toxicities related to the second transplant regimen. The signifi.

Sensitivity analyses Symptom severity at baseline Information was pooled from studies that included men with mild plus moderate symptoms at baseline, and was compared with pooled information from studies including only men with at least moderate symptoms at baseline. Only two trials included men with mild symptoms, with fewer than 500 per group. There was insufficient information to determine whether symptom severity at baseline affected treatment efficacy in terms of prostate volume or urinary flow rate. Prostate volume at baseline The sensitivity of results to prostate volume was investigated in two ways. In the first, results were pooled according to whether mean prostate volume of the finasteridetreated group at baseline was less than 40 cm3 or 40 cm3 or greater. Prostate volume and maximum urinary flow.

Because of insufficient cell numbers, we were not able to perform clonality studies by X-chromosome inactivation on hematopoietic progenitors at time of diagnosis. However, bone marrowimmunophenotyping at that moment has shown the presence of high numbers mean, 18%; range, 12% to 27% ; of CD34' myeloblasts in four patients UPN 1, 3, 4, and 5 ; . Therefore, it seems very unlikely that any remaining nonclonal hematopoietic progenitors could be easily detectable in these CD34' blastic fractions.

The provisions, test and assessment methods in this guideline or referred to, have been written, based upon the assumed intended working life of the product for the intended use of 50 years, provided that the product is subject to appropriate use and maintenance cf. Ch. 7 ; . These provisions are based upon the current state of art and the available knowledge and experience. An "assumed intended working life" means that it is expected that, when an assessment following the ETAGprovisions is made, and when this working life has elapsed, the real working life may be, in normal use conditions, considerably longer without major degradation affecting the essential requirements. The indications given as to the working life of a product cannot be interpreted as a guarantee given by the producer or the approval body.
