Objectives: To investigate the mechanism of ciprofloxacin resistance in isolates of Acinetobacter baumannii during two hospital outbreaks and to determine the expression level of the gene encoding the AdeB efflux pump. Methods: Isolates were previously typed by PFGE and their MICs determined by broth microdilution. The gyrA and parC genes were sequenced and the adeB gene examined by real-time reverse transcription PCR RT-PCR ; . Results: Two clonal lineages were responsible for the two hospital outbreaks. In both outbreaks, ciprofloxacin susceptibility was reduced during the course of the outbreak when compared with the index isolates. Mutations in gyrA and parC were found to have occurred during the outbreak. The MICs of non-fluoroquinolone antibiotics were raised in one clonal lineage and this was associated with a 10fold increase in mRNA transcripts for adeB. Conclusions: We have witnessed the appearance of gyrA and parC mutations during outbreaks of A. baumannii. In parallel with these mutations, we observed up-regulation of the adeB gene associated with a decrease in susceptibility to non-fluoroquinolone antibiotics. These data illustrate the propensity for A. baumannii to develop multi-drug resistance rapidly. Keywords: ciprofloxacin, efflux, RT-PCR, multidrug resistance.

The fundamental process of labour is progressive dilatation of the cervix. The woman herself usually diagnoses labour when she has recurrent painful uterine contractions. However, such contractions may be ripening the cervix the latent phase ; before rapid cervical dilatation the active phase ; occurs. Midwives and doctors judge progress by assessing the descent of the fetal presenting part on abdominal palpation and advancement of the fetus on vaginal examination position of the presenting part relative to the ischial spines ; . These may be imprecise measurements, but a series of careful assessments by the same observer is usually informative.

Damage Liu et al., 2000 ; . In analogy to the cell culture observations, in this study, injection of LPS to the SN of rat brain resulted in a rapid activation of microglia at 6 h after the LPS injection. However, the loss of TH-positive neurons was only observed 3 days post-LPS injection. This temporal relationship between microglial activation and neurodegeneration strongly suggests that microglial activation precedes the degeneration of dopaminergic neurons in the SN. Creased in atherosclerotic lesions.1 PAF is a phospholipid with proinflammatory and thrombogenic properties. PAF levels were found to be higher in the coronary arteries from some patients with severe atherosclerosis.6 However, the levels of PAF in early atherosclerotic lesions are not known. There are several reports demonstrating the generation of ether- and ester-containing PAF-like lipids during oxidation of LDL79 and polyunsaturated fatty acidcontaining phospholipids.1013 Smoking, a risk factor for atherosclerosis, has been shown to cause an increase in the levels of PAF and PAF-like lipids in plasma lipoproteins.14 tk; 2Monocyte entry into the vessel wall has been shown to play an important role in atherogenesis, and in vitro studies have demonstrated the effects of phospholipid oxidation products and PAF on monocyte endothelial cell interac.

Arterial reactivity was studied using high-resolution external vascular ultrasound of the right brachial or radial artery, as described by Celermajer et al. 16 ; , using a GE-Vingmed System 5 mainframe and a 10-MHz linear array transducer. Arterial diameter was measured at rest, during reactive hyperemia leading to FMD, an endothelium-dependent stimulus ; and after a 400- g spray of sublingual nitroglycerin GTN, an endothelium-independent dilator ; . Reactive hyperemia responses were assessed following temporary 4.5 min ; brachial artery occlusion with a sphygmomanometer placed below the target artery cuff to 250 mm Hg ; . Comparing arterial diameter measurements during reactive hyperemia to baseline allows calculation of the values for FMD, which is predominantly due to endothelial nitric oxide release 17 ; . Analysis was carried out by two independent observers. Operators and analyzers were blinded to treatment assignment, and analyzers blinded to the stage of the experiment. Intima-media thickness IMT ; was measured using the same ultrasound machine and transducer, for both the left and right common carotid arteries. The image was focused on the posterior wall, and images of the distal 10 mm of the common carotid artery was recorded from the angle showing the greatest distance between the lumen-intima interface and the media-adventitia interface, as previously described 18 ; . The distance between the characteristic echoes from the lumen-intima and the media-adventitia interfaces was taken as the measure of IMT 19, 20 ; . Scans were analyzed with a previously validated computerized edge detection system 21 ; . Two end-diastolic frames were selected, digitized and analyzed for maximum and mean IMT; next, the average reading was calculated from these two frames and from the average of left and right arteries 18.

A photosensitizing drug is typically administered systemically through intravenous injection or, in some cases, applied topically to a cutaneous lesion.3 After the administration of the photosensitizer and before the application of light, a waiting period the drug-light interval ; of several hours to several days is usually imposed in order to allow optimal distribution of the photosensitizer between malignant and normal tissue. After systemic delivery of photosensitizer, an internal lesion can be accessed surgically or endoscopically. With traditional PDT, laser light is then delivered through an optical fiber to the surface of a thin lesion, or interstitially from within tumor parenchyma to a thicker lesion.4, 5 Cutaneous tumors are typically treated with a light dose of 10300 J cm2 , administered over a period of minutes. The patient then avoids exposure to strong indoor light and sunlight for a variable photosensitizer-dependent ; time period to prevent a cutaneous phototoxic reaction and gabitril.
Nance spectroscopic studies in schizophrenic subjects have demonstrated significant reduction of NAA levels in these same regions 37 ; . However, the factors that regulate brain NAA levels are complex 38 ; . While these findings suggest diminished activity at glutamatergic receptors in relevant brain regions, it remains uncertain whether this represents a primary vulnerability factor or a compensatory response to a more proximal defect. Ligand binding studies in postmortem brains from individuals with schizophrenia have revealed consistent increases in kainate receptors in the prefrontal cortex 39, 40 ; and decreased AMPA and kainate receptor binding in the hippocampus 41, 42 ; without consistent abnormalities in NMDA receptor density 3, 4 ; . Immunocytochemical analyses have confirmed a decrease in AMPA receptors in the medial temporal lobe, although reductions in the hippocampus were not found in one study 3 ; . Whereas ligands binding to the cation channel of the NMDA receptor complex "PCP binding site" ; have not demonstrated consistent alterations in density 3 ; , Ishimaru et al. 43, 44 ; reported increased binding to the glycine site of the NMDA receptor throughout the primary sensory cortex and related association fields. In addition, the binding of [3H]-Daspartate, which labels the transporters that remove synaptic glutamate, was increased in the frontal cortex and decreased in the striatum 39, 45 ; . The cloning of receptor subunits has facilitated the measurement of glutamate receptor expression in the brain. Most consistent has been the finding of a lower level of mRNA encoding AMPA receptor subunits in the hippocampus and parahippocampus of schizophrenia brains than in the brains of comparison subjects 3, 46, 47 ; . In addition, Akbarian et al. 48 ; found a higher proportion 1% versus 0.1% ; of unedited GluR2 mRNA in the prefrontal cortex of patients with schizophrenia and Alzheimer's disease than in normal subjects, suggesting a higher level of permeability of AMPA receptors to calcium, which could increase the potential for neurotoxicity. Although less studied, the measurement of mRNA encoding kainate receptor subunits has demonstrated a similar pattern of lower density in the hippocampus and parahippocampus 3 ; . Although ligand binding studies have generally failed to find altered NMDA receptor density in the brain in schizophrenia, two studies have found evidence of altered subunit composition of NMDA receptors. Akbarian et al. 48 ; found a relatively higher level of the NR2D subunit in the prefrontal cortex of schizophrenia subjects than in normal subjects and neuroleptic-treated comparison patients, suggesting increased potential responsiveness to glutamate. In contrast, Gao and colleagues 4 ; recently reported a relative decrease of the NR1 subunit in the hippocampus of schizophrenia patients. The investigators demonstrated that this finding was unlikely to represent a medication effect since treatment with haloperidol for 6 months produced no effect on NMDA receptor subunit composition in the rat hippocampus 4 ; .
