There was a 7 year history of secondary infertility following a previous pregnancy which was terminated in the first trimester in 1988. Endometriosis was diagnosed at a laparoscopy and hydrotubation performed in 1989. She subsequently received danazol therapy. In 1992 a left salpingo-oophorectomy was performed after the development of ovarian endometrioma. Cryptococcus neoformans causes infection mainly in immunocompromised hosts. Current treatment regimens for cryptococcal meningitis are based on amphotericin B, with or without flucytosine. This treatment is followed by maintenance therapy with fluconazole which has been related to the development of antifungal resistance.1 The mechanisms of azole resistance described in C. neoformans include enhanced energy-dependent drug efflux2 and point mutations in the ERG11 gene which are responsible for the amino acid substitution glycine 484 for serine G484S ; .3 Resistance in C. neoformans clinical isolates remains uncommon and has not increased in the last decade in the United States and the UK, 1, 4 but local increases in resistance have been reported in other areas such as Cambodia.5 These studies have shown geographic differences in species distribution and antifungal resistance. Recently, Pfaller et al.4 reported a 15 year survey including susceptibility data on 1811 clinical isolates of C. neoformans and goldenseal.
