The medical charts of all the patients who had a diagnosis of IPH and received medical care at Childrens Hospital Los Angles from January 1972 to March 1998 were reviewed. The diagnosis of IPH was established by evidence of recurrent pulmonary hemorrhage hemoptysis, infiltrates on chest radiograph, presence of hemosiderin-laden macrophages in BAL or in gastric aspirates, or typical microscopic findings in lung biopsy ; , hypochromic microcytic anemia, and the absence of other diseases that can result in pulmonary hemorrhage. All patients underwent either a lung biopsy or an extensive diagnostic evaluation to exclude secondary causes of pulmonary hemorrhage, including vasculitides, rheumatologic diseases, immune deficiency, and glomerulonephritis. The following data were collected: age at diagnosis, sex, clinical presentation, method of diagnosis, treatment, and clinical course during subsequent follow-up. All patients received long-term follow-up at our institution and did not develop any evidence of extrapulmonary disease. When available, autopsy reports were reviewed to determine the cause of death. Informed consent was obtained from all patients and the study was conducted according to the tenets of the Declaration of Helsinki. Patients who had any form of CB in our classification system thus all types were included ; for at least 6 months were carefully evaluated.1, 9 The patient population was 56% male n 61 of this male population, CB-KCS was present in 39% n 24 ; and CB-KCS was present in 50% n 23 ; of the female population. If the patients with a staphylococcal infection were omitted, the occurrence of CB-KCS in the female population was 38% n 11 ; and 41% n 23 ; in the male population. In a previous study, we published2 this patient population's association with decreased levels of production of tear lysozyme in the CB-KCS patients as compared with the CB patients. Of these patients in the present study n 19 ; , 7 had a clinical picture consistent with KCS that persisted after the CB had been controlled and treatment with ocular medications discontinued, ie, interpalpebral epithelial erosions with staining of conjunctiva and cornea with a vital stain rose bengal ; , and decreased tear meniscus on slitlamp examination. None of these patients had a general meibum deficiency. Relatedly, it has been observed that lacrimal gland function is decreased in patients with KCS and increased evaporation.8 Meibomian gland secretions were collected as follows. After instillation of 1 drop of lidocaine onto the ocular surface, a lid conformer was placed into either the inferior or superior cul-de-sac. A sterile cotton swab was then passed over the lid margin to remove excessive debris and tears. The tarsal plate containing the meibomian glands was then squeezed between the swab and conformer. The secretions were harvested with a sterile platinum spatula. Samples were washed off the spatula with chloroform, collected in glass tubes, and evaporated to dryness with nitrogen. Tubes were then sealed under nitrogen ; and stored at -70C. Lipid classes were first separated by thin-layer chromatography as previously described.10 Samples were dissolved in chloroform and spotted on silica gel H plates, previously activated at 120C. The plates were developed in hexanediethyl etheracetic acid 75: 25: 1 ; , the region of origin containing the polar lipids was scraped off, and then it was placed in nitrogen-flushed screw-capped tubes.

The question of polymorphisms in ENaC subunits has recently received extensive attention because of possible association with some hereditary forms of hypertension 20 ; . In Table 4 we present the list of all single nucleotide variants observed in all three genes encoding ENaC -, -, and subunits. Only one variant we observed in the -subunit gene changes the Thr663 codon to Ala. This variant has been previously reported in different individuals 29, 3234 ; , indicating that it represents a genuine single nucleotide polymorphism SNP ; . Yet, its biochemical significance remains unknown. In the -subunit gene we have identified additional sequence differences in the untranslated region of exon 2 and in the introns of the gene compared with previous reports. As some of these differences were observed in all of our subjects from different ethnic groups, it is possible that previous sequences based on a single report may contain sequencing errors at some of these bases. In the -subunit gene we observed only two different single nucleotide changes. The functional significance, if any, of the amino acid change remains to be determined. Several previous studies examined partial sequences of - and subunits only selected exons in the carboxyl-terminal region ; 3537 ; in hypertensive subjects. Although Persu et al. 35 ; identified novel rare SNPs, no mutation was found to be definitively associated with hypertension 3537 ; . Exons 8 and 12, noted in a previous study 35 ; , are here numbered 9 and 13, as we now know that there are 13 exons in the ENaC gene 9 ; , not 12. We sequenced the complete coding sequence of the ENaC gene GenBank accession no. AF356502 ; from six individuals from different ethnic origins, to resolve the discrepancies in the previously published ENaC complete mRNA sequences 7, 32 ; and partial genomic sequences 30 ; . In our subjects we observed complete conservation of the coding sequence. The single nucleotide change we observed was located in the 5 -UTR Table 4 ; . Yet, we detected eight single nucleotide differences, five of which also change the amino acid sequence compared with previously published sequences Table 4 ; . Because of the discrepancies among previously published mRNA and genomic sequences themselves, it is likely that some or all of the differences we observed may reflect sequencing errors, rather than genuine SNP. As we sequenced six individuals from different groups, we suggest that the amino acid sequence based on our genomic sequencing be used as the standard ENaC sequence rather than the previously published sequences with the differences listed in Table 4. In summary, our sequencing of multisystem PHA patients revealed novel homozygous mutations in the and ENaC genes that should result in reduced or abolished ENaC activity. Consistent with the autosomal recessive inheritance of the disease, the parents of at least two of the patients were heterozygous for the mutation. The present results revealed only a few sequence variants among individuals from dif and meclizine.

N-Alcohols exert a dual action on neuronal nicotinic acetylcholine ACh ; receptors with short chain alcohols exhibiting potentiating action and long chain alcohols exhibiting inhibitory action. n-Butanol lies at the transition point from potentiation to inhibition. To elucidate the mechanism of dual action of alcohols, the effects of n-butanol on the human 42 ACh receptors expressed in the human embryonic kidney 293 cell line were analyzed in detail by the whole-cell patch clamp technique. Prolonged applications of nbutanol evoked small currents with an EC50 of 230 90 mM and a Hill coefficient of 1.8 0.4. This current was blocked by either the ACh channel blocker mecamylamine or the receptor blocker dihydro--erythroidine indicating that butanol activated receptors as a partial agonist. As expected from its partial agonist action, n-butanol also modulated ACh-induced currents in a concentration-dependent manner. Butanol at 300 mM potentiated currents induced by low concentrations of ACh 30 M ; , while inhibiting the currents induced by high concentrations of ACh 100 - 3000 M ; . In addition, butanol at a low concentration 10 mM ; suppressed the currents evoked by 10 - 3000 M ACh, a result consistent with a channel blocking action. Most features of n-butanol effects were satisfactorily simulated by a model in which butanol acts as a partial agonist and as a channel blocker and mefloquine.
