We characterized the effects of single and combination antifungal therapy on survival, histopathology, and laboratory markers of fungal burden in experimental aspergillosis in the p47phox knockout mouse model of CGD. CGD mice were highly susceptible to intratracheal Aspergillus fumigatus challenge, whereas wild-type mice were resistant. CGD mice were challenged intratracheally with a lethal inoculum 1.25 104 CFU mouse ; of A. fumigatus and received one of the following regimens daily from day 0 to 4 after challenge n 19 to per treatment group ; : i ; vehicle, ii ; amphotericin B intraperitoneal; 1 mg kg of body weight ; , iii ; micafungin intravenous; 10 mg kg ; , or iv ; amphotericin B plus micafungin. The rank order of therapeutic efficacy based on prolonged survival, from highest to lowest, was as follows: amphotericin B plus micafungin, amphotericin B alone, micafungin alone, and the vehicle. Lung histology showed pyogranulomatous lesions and invasive hyphae, but without hyphal angioinvasion or coagulative necrosis. Treatment with micafungin alone or combined with amphotericin B produced swelling of invasive hyphae that was not present in mice treated with the vehicle or amphotericin B alone. Assessment of lung fungal burden by quantitative PCR showed no significant difference between treatment groups. Serum galactomannan levels were at background despite documentation of invasive aspergillosis by histology. Methods: Medical Records of 667 hospitalized elderly patients 65 year old ; were reviewed retrospectively since March 2003 to March 2004. Data were analyzed by SPSS 12. Results: Mean age of patients was 75 years. Seasonal pattern demonstrated that most of admissions were in summer 27.6% ; followed by autumn 27.3% ; , spring 25% ; , and winter 20.1% ; . Most patients with respiratory tract infections 30.4% ; , meningitis 57.1% ; and tuberculosis 30.4% ; were admitted in autumn. Most patients with urinary 31.5% ; and gastrointestinal tract 32.7% ; infections, and endocarditis 66.7% ; were admitted in spring. Sepsis 54.8% ; , septic arthritis 30.8% ; , and hepatitis 36.4% ; cases admitted mostly in summer and skin-soft tissue infections mostly in winter 31.0% ; . Common causes of hospitalization in infectious wards were as follows respectively: Respiratory tract infections 41.4% ; , Urinary Tract Infections 21.9% ; , sepsis 9.3% ; , Gastrointestinal Infections 7.8% ; , non-infectious etiologies 4.9% ; , Skin and Soft tissue Infections 4.3% ; , tuberculosis 3.4% ; , Septic Arthritis 1.9% ; , Hepatitis 1.6% ; , Meningitis 1% ; , Fever of Unknown Origin 0.9% ; , expired before diagnosis 0.9 ; and Endocarditic 0.4% ; . 92.1% of admitted cases were discharged and 7.9% expired during hospitalization. Sepsis was the most common infectious cause of death 45.2% ; . Conclusion: Respiratory tract infections are the most common causes of hospitalization in infectious wards in elderly population of Iran. Infectious wards are mostly crowded with elderly patients in summer and autumn. Mortality 45.2% due to sepsis ; is directly related to the etiology of hospitalization. These results would mandate special care for prevention of respiratory tract infections such as vaccination and planning a special policy for hospital beds required for elderly patients in Iran and micafungin.
