Showed promise in pilot studies that involved small numbers of patients. Until the results of definitive trials are available, none of these interventions can be recommended for use in patients with HF. Several drugs that showed promise in pilot studies and were included in this section in the 2001 guidelines failed to live up to their promise in long-term, largescale trials and are no longer included as "promising" in this update. Several remain under or have begun active investigation. Investigational drug therapies currently in phase III evaluation for the treatment of chronic HF include vasopressin receptor antagonists, intermittent nesiritide infusions, and oral phosphodiesterase III inhibitors. In addition, newer devices and technologies, such as implantable hemodynamic monitors and internal cardiac support devices, external counterpulsation, treatment for sleep-disordered breathing, myocardial growth factors and stem cell transplantation, and devices to achieve intravascular volume reduction, as well as novel surgical approaches, including surgical ventricular restoration, are under active investigation. Several of these are discussed below. 4.3.1.4.1. TECHNIQUES FOR RESPIRATORY SUPPORT. Patients with HF frequently exhibit abnormal respiratory patterns, including Cheyne-Stokes breathing and sleep-disordered breathing 398 ; . In the Sleep Heart Health Study, the presence of sleep-disturbed breathing was associated with a 2.38 relative risk of HF independent of other known risk factors 399 ; . This risk of HF exceeded that for all other cardiovascular disease syndromes evaluated, including hypertension, stroke, and coronary artery disease. The use of nocturnal oxygen and devices that provide continuous positive airway pressure has been reported to produce symptomatic improvement 400, 401 ; . Although there is no direct evidence that treatment of sleep-disturbed breathing prevents incident HF, treatment of established LV dysfunction with continuous positive airway pressure breathing has been shown to improve LV structure and function in patients with either obstructive or central sleep apnea disturbed-breathing syndrome 402 ; . Additional studies are in progress to evaluate the efficacy of these interventions. It is hoped that such studies will provide information about the efficacy and safety of this approach and help identify patients most likely to benefit from treatment. 4.3.1.4.2. EXTERNAL COUNTERPULSATION. The technique of external counterpulsation involves the use of a device with inflatable cuffs that surround the lower limbs and inflate and deflate in synchronization with the cardiac cycle. The device is designed to reduce loading conditions in systole while increasing coronary perfusion pressures in diastole 403 ; . External counterpulsation has been shown to reduce the frequency and severity of anginal attacks in patients with symptomatic coronary artery disease 404 ; . A possible mechanism of action for this observed clinical effect may be an improvement in endothelial function of the coronary vascular bed 405, 406 ; . Early trials of this therapy in patients with HF and low EF have been encouraging, and a randomized trial.

In a 2000 report, Colucci and colleagues concluded that "nesiritide would be a valuable addition to the initial treatment of patients admitted to the hospital for decompensated congestive heart failure."1 But their placebo-controlled, dose-ranging trial was focused on short-term monitoring of the pulmonary-capillary wedge pressure. Follow-up data on these subjects, which were not part of the study design as reported in that article, suggested that nesiritide may have had an adverse effect on 30-day mortality, which was 7.1 percent in the nesiritide group, as compared with 4.8 percent in the placebo group P 0.62 ; .2 Over this longer period of follow-up, the incidence of substantial de. UNISA I was very happy to receive a copy of "The Military Chaplain". Congratulations on the publication of this new magazine. It is attractive, interesting and informative. In an everincreasing secular environment it is good to see how seriously the National Defence Force takes the role of the religion in public service. In a country such as ours where the churches employ so many people in the social service sector of society it is important to recognise and indeed highlight the role of religion in our lives. Your publication certainly does that very well. I hope to read many more issues of it. Dr. Madge Karecki Senior Lecturer, Dept. Missiology: Unisa AIR FORCE GYMNASIUM I recently attended the training course for facilitators of the Cpln General's project on Combating HIV AIDS through spiritual and ethical conduct, at Rhemardo. This is now to be implemented throughout the SANDF, a responsibility which rests squarely on the shoulders of each and every chaplain. We, more than anyone else except perhaps for the medical personnel ; , know the devastating effects which this pandemic is wreaking in the lives of so many of our members. We as chaplains have a unique opportunity to make a difference. We dare not let this opportunity pass us by. This programme represents a kairos moment moment of truth ; for us in the Chaplains Service. Now more than ever, we can prove to our critics that we do indeed add value, that we are playing a pivotal role in the fight against this scourge. Yes, the programme is not perfect. It has its flaws and there is criticism which one may level against it - this column may even be the appropriate forum through which we can voice our concerns. We cannot, however, allow the flaws in the programme or our criticism of it to become an excuse for lethargy or immobility in its implementation. We can adapt and contextualise the programme as we become more familiar with its contents. We can make it work. The bottom line is that we must make it work. We have no other option if we wish to make the difference that God calls on us to make at this time. Cpln Rev ; A.J. Treu Air Force Gymnasium.

Of interest, the inhibitory effects on DC phenotype were not mediated by reduced amount of nuclear RelB protein data not shown ; , a transcription factor of the NF- B family important for DC development.22 In order to further analyze the effects of ESE-3 downregulation, secretion of cytokines known to be important for DC function was analyzed by ELISA. While no effect on IL-12 p70 ; , TNF- , and IL-10 production was detected data not shown ; , IL-6 production was increased in cells transfected with siRNA against ESE-3 Figure 6 ; . To further analyze the possible physiologic significance of increased IL-6 production, we examined the phenotype of DCs and nexavar.
