Van Ingen Schenau and colleagues 2426 ; have studied the functional roles of mono- and biarticular muscle in human jumping and cycling motions. They have proposed two unique roles for biarticular muscles: 1 ; power produced by a monoarticular muscle can be transported to adjacent joints by its biarticular antagonist and 2 ; the directional control of externally applied forces. Furthermore, the coordination of related monoand biarticular muscles is at least partly under the influence of geometric constraints 24, 25 ; . In cycling, an alteration of grade and a change in posture result in changes in the direction of the force applied to the pedal 7, 8 ; and may alter the influence of geometric constraints in the leg. Thus the relationship of activity patterns between monoarticular muscles and their biarticular antagonists may well be modified under these conditions. Therefore, muscle activity patterns during uphill cycling in both seated and standing postures are of interest. Changing grade will modify the line of action of the gravitational force in relation to the forces exerted on the pedal by the cyclist. Altering posture to standing will not only change the relations of the forces concerned but will also change the geometry of body segments during the motion. What is the impact of these changing relationships of the forces on muscle coordination? How does the neuromuscular system adapt to the change of the body geometry? I couldn't help chuckling when I saw how disappointed the man looked as he departed. Tightfisted, that's what my old man was. Even with so much cash coming in, my father would lose it all gambling and have to turn to my mother for help. It usually went something like this. But her husband proved to be a gifted actor in this situation, too. Picture the scene: my father comes home one day, with no prior notice, after a year away. Receiving him in the entrance hall, my mother keeps her cool until she is through exchanging the usual formalities. Then, just as she begins to lay into him, he raises his hand in an effort to hold back her rising anger. "Hey, listen. I met this woman who looked just like you. So in spite of myself, I . well, you can imagine the rest." Mother used to tell me that his performance was exquisite and his timing perfect. Struck by the ridiculousness of the situation, she didn't feel like arguing with him further, but would shut him up by telling him she didn't want to hear any more of his nonsense. At other times, in a fit of rage at her husband's behavior, she tried to get him to sever his yakuza ties. I imagine she wondered what the point.
In July 1995, a 30-yr-old man was admitted to a department of general internal medicine because of headaches, nausea, and a blood pressure of 230 130 mm Hg. Hypertension had been diagnosed in 1993, but the patient took antihypertensive drugs only occasionally. On admission in 1995, his serum potassium was 2.8 mmol L, sodium was 142 mmol L, and creatinine was 106 mol L. His body mass index was 26 kg m2. Apart from the hypertension, physical findings were normal. Funduscopy showed slightly narrowed retinal arteries without bleeding or papilledema. Hyperaldosteronism was suspected as the underlying disorder. After potassium chloride supplementation, urinary aldosterone excretion was slightly increased 47 nmol day ; . In the postural stimulation test plasma aldosterone levels were 0.89 and 1.22 nmol L 32 and 44 ng dL ; , and PRA levels were 0.44 and 0.90 ng mL h the supine and upright positions, respectively. The plasma aldosterone PRA ratios were elevated Table 1 ; . Renal arteriography, urinary catecholamine excretion, and adrenal computed tomography yielded no pathological findings. Selective adrenal venous catheterization was technically unsuccessful. The patient was discharged with the recommendation to take spironolactone and antihypertensive drugs. In 1997 he suffered a stroke with right-sided brachiofacial paresis and motoric aphasia after having taken his recommended antihypertensive drugs including spironolactone ; only occasionally. He recovered completely and stopped taking the drugs in October 1998. In April 1999 he was admitted to this unit because of muscle weakness, dizziness, and sweating with a blood pressure of 230 160 mm Hg and a serum potassium of 2.2 mmol L. His serum sodium was 144 mmol L, and creatinine was 111 mol L. Urinary aldosterone excretion was markedly elevated at this time 291 nmol day ; . Antihypertensive therapy with doxazosine and potassium supplementation reduced his blood pressure to 160 200 90 mm Hg. The postural stimulation test, adrenal computerized tomography, and adrenal venous catheterization were repeated because the very low serum potassium level indicated the presence of an adrenal adenoma 2, 7 ; . The postural stimulation test yielded supine and upright values of plasma aldosterone of 3.33 and 2.86 nmol L 120 and 103 ng dL ; , and PRA levels of 2.5 and 4.2 ng mL h, and plasma aldosterone renin ratios were elevated Table 1 ; . This time, bilateral adrenal venous catheterization was successful, as and pramlintide.

Incident Pain and Bone Pain 1. Bone pain from cancer metastases is exceedingly common. Bone metastases are a significant source of morbidity with decreased mobility and function and pathological fractures. 2. Bone pain is well localized, dull and constant in character with sharp flares with movement or pressure incident pain ; . There may also be associated muscle spasm. Bone pain is often worse at night. 3. Incident pain can be defined as an intermittent exacerbation of pain triggered by movement, weight bearing or increased pressure or procedures such as dressing changes. 4. May get incident pain at times from other sources such as nerves tethered by tumour, large tumours that cause pressure phenomena on movement or with upright posture or sensitive skin edges on skin ulcers and prevnar.
