The problem is that neurology is a big field covering lots of diseases so not all neurologists deal with MS patients. Of the 140 neurologists Piper Jaffray surveyed, only 38 27% ; had any firsthand experience prescribing Tysabri. Because Piper put all 140 neurologists in the same bucket, their report gives the impression that 41% of knowledgeable doctors don't believe Tysabri should return to market and 8% don't even think it's a valuable therapy. I don't know how knowledgeable about Tysabri the other 102 neurologists are, so while I respect their opinions, I would give them somewhat less weight than the 38 with firsthand experience. Imagine how different Piper's conclusions would have been if it turned out that 100% of the 38 doctors with firsthand experience believed Tysabri is a valuable therapy for MS and should be returned to the market. This may in fact have been the case, but you can't tell from what's been published. In my Elan Research Alert on 1 23 said that Wall St. is not giving MS patients enough credit. The MS community is very organized, knowledgeable, and networked together and I have not seen any Wall St. analyst recognize the power or importance of that. Of course physicians will respond to a survey that they would like to know more about the risks of PML and have their liability reduced before prescribing it. That is always the case. However, when your MS patient is willing to give up all other drugs and suffer the painful effects of MS so that they can be ready to take Tysabri as soon as it is re-approved, will you tell them to wait for more data? I don't think so. Piper has the right idea but I'm not sure they asked the right questions to the right people. Even if Piper is right, and doctors prescribe Tysabri for 10% of MS patients that would be 40, 000 patients in the U.S. and another 40, 000 in Europe. At , 500 per patient, we're talking about nearly billion a year in revenues just for MS not including crohn's disease ; . That's a blockbuster success by anyone's definition. Tysabri is scheduled to be discussed at a FDA advisory committee on March 7th and 8th and the FDA's decision is due March 26th. I expect Elan's stock price to be very volatile as everyone who will be hurt by Tysabri's return to market will be sure to put the most negative spin they can on Tysabri's clinical data. When the dust settles, I think Wall Street already agrees with us that Tysabri will be reapproved so I do not expect a quick pop upon FDA approval. The disagreement now is about how fast sales will ramp. If Kelly Martin, Elan's CEO, is right about having 25, 000 patients waiting to get back on Tysabri, sales could ramp quickly. Wall Street's expectations for Tysabri's sales are currently set so low that that this initial surge of patients could provide a big enough upside surprise to Wall Street to drive Elan's next double.
